The exact mechanism of action of Mesalamine is unknown, but it appears to work topically rather than systemically. In patients with chronic inflammatory bowel disease, the mucosal production of arachidonic acid metabolites is increased through cyclooxygenase (prostanoids) and lipoxygenase (leukotrienes and hydroxy eicosatetraenoic acids) pathways. Mesalamine reduces inflammation by blocking cyclooxygenase and inhibiting prostaglandin production in the colon.

Using Mesalamine alongside nephrotoxic agents like NSAIDs and Azathioprine may increase the risk of kidney problems. Mesalamine should not be used by individuals with hypersensitivity to salicylates or any component of the formulation.
Tablet Dose:
Treatment of mild to moderate Ulcerative Colitis
Adults: 800 mg (two 400 mg tablets) three times daily for 6 weeks. The dose may be increased up to 4.8 g/day in divided doses if necessary.
Children (5 years and older):
Maintenance of remission in mild to moderate Ulcerative Colitis
Maintenance of remission in Crohn’s Disease

Mesalamine should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus. Caution is advised for nursing mothers.
Patients may experience delayed gastric emptying, which can delay Mesalamine release in the colon.
Cases of nephropathy and interstitial nephritis have been reported. Renal function should be checked before starting treatment and monitored periodically.
